There are two methods to create manholes in a diaphragm.
[Select Vertex] & [Select Edge]
Method to Create Diaphragm Manholes [Select Vertex]
Create diaphragm manholes using the Select Vertex method.

Select edges to create a closed diaphragm manhole.
